What is the OCF-23?
The OCF-23 (Treatment Confirmation Form) is used when treating patients under the Minor Injury Guideline (MIG). Unlike the OCF-18, this form does not require insurer pre-approval—it simply notifies the insurer that the patient is receiving care under the standard MIG coverage.
When to Use the OCF-23
✅ The patient’s injuries fall under the MIG (e.g., sprains, strains, whiplash).
✅ The patient does not require extensive assessments or treatment beyond the MIG cap ($3,500).
✅ You’re providing pre-approved physiotherapy, chiropractic care, or massage therapy.
Key Sections to Fill Out
1️⃣ Patient & MVA Details – Insurance information and date of accident.
2️⃣ Provider Details – Your name, clinic information, FSCO number.
3️⃣ Diagnosis and Injury Details – Select the appropriate WSIB injury codes.
4️⃣ Treatment Plan – Outline what you’re providing under the MIG, including expected duration and goals.
5️⃣ Patient Consent & Signature – Ensure the patient understands their treatment plan.

The MIG covers a maximum of $3,500 for treatment.
