What is the OCF-18?

It outlines your proposed treatment plan and must be approved by the insurer before treatment can begin—unless the patient falls under the Minor Injury Guideline (MIG).
Who Needs to Complete It?
This form is required if:
✅ The patient does not fall under the MIG and needs treatment beyond the standard minor injury coverage.
✅ The patient requires assessments or additional treatment outside of the MIG limits.
✅ You need approval for services that exceed the pre-approved framework (e.g., extended physiotherapy, chiropractic care, psychological services).
Key Sections to Fill Out
1️⃣ Patient Information & MVA Details – Basic patient details, accident date, and insurance provider.
2️⃣ Provider Information – Your clinic’s name, contact details, and FSCO number.
3️⃣ Proposed Treatment Plan – Be specific about:

Frequency and duration of care (e.g., 2x/week for 6 weeks).

Tips for Faster Approval
